Please fill out the Membership Application Form and either fax or mail to COTAPSA's office.
Once COTAPSA receives your application, we will notify the payroll department to start payroll deductions. COTAPSA dues are $10.00 per pay period - $260.00/yr. Dues are tax deductible. Once we see that payroll deductions for dues have started, we will send you a welcome letter. Your membership begins the day we receive your application.
